If these inputs are open, shorted, incorrectly rated, or improperly connected, the relay may not detect the fault condition and will fail to send a trip signal . 2. Incorrect Relay Settings or Coordination If the relay pickup settings, time delays, or coordination with upstream/downstream devices are incorrect, the relay may not operate as intended. Misconfigured settings can prevent the relay from recognizing a fault or delay its response beyond the breaker's capability . 3. Trip Circuit or Breaker Issues The relay sends a trip signal to the breaker through a trip circuit. If the trip circuit is open, has blown fuses, or suffers from poor connections, the breaker will not receive the trip command. Similarly, a malfunctioning breaker or its trip coil can prevent isolation of the faulted section . 4. Master Trip (Lockout) Relay Malfunction The master trip relay (ANSI code 86) acts as an intermediary between protection relays and the breaker trip coil. If this relay fails, the trip command from the protection relay cannot reach the breaker. Common issues include de-energized operating coils, stuck contacts, or improper wiring . 5.
